WHAT FORM DO I NEED TO GIVE MY DAUGHTER POA FOR MY FINANCES WHEN I DIE OR BECOME INCAPACITATED?

Thank you for the question. You need to have a Financial and Health Care Power of Attorney prepared by a local attorney in order to give your daughter control should you become incapacitated. You should also have a Living Will and Last Will and Testament prepared. Typically the Power of Attorney is prepared at the same time as a Will. There are resources online that will allow you to self-prepare these documents. If you use this type of resource, make sure that all documents are signed and witnessed properly.
